Please have any previous x-rays forwarded to our office.
Please bring a list of any current medications you are taking.
Please bring current and accurate dental insurance information.
Please alert us to any medical condition that may alter or change your dental care.
Cancellation: Should you need to reschedule or cancel your appointment, kindly give us 48 hours notice.
Financial Policy: For your convenience, we accept almost all insurance carriers and all major credit cards. Payment is due in full at the time service is rendered. Any remaining balance after your insurance has paid is your responsibility. Call to find out about our various financing offers.
